项目摘要

2019年度実施した調査①（社会的ハイリスク妊婦の支援を行っている医療機関において妊娠期に提供された支援内容を診療録からデータ収集）の分析中である。面談では、抱える社会的課題の状況や身体・精神症状の有無を確認し、それらの症状に影響を与える要因・誘因を探っていた。妊婦が安心して気持ちを表出できる場を確保し、不安・不満を受け止めていた。抱える社会的課題を整理するために積極的傾聴を行い、支援の手掛かり探っていた。妊婦の成育歴を知り、抱える社会的課題の経緯と妊婦の捉え・社会的課題に関連した具体的なエピソードを傾聴しながら、妊婦の思考・認知・行動パターンを把握することで、妊婦の個別性を捉えようとしていた。その上で面談を通して医療者が感じたことや整理したことを妊婦にフィードバックしていた。そして、抱える社会的課題が妊娠・出産・育児に与えるであろう影響をアセスメントし、妊婦と共に取り組んでいく課題であるという共通認識を持ち、対応を検討していた。さらに妊婦自身の社会的課題への対処行動を確認し、妊婦の持っている力や解決しようと努力する姿勢を認め、ねぎらいながら妊婦自身が実践できそうな具体的な助言を行っていた。そして、夫や原家族との関係性も確認し、キーパーソンを特定していた。夫や家族の育児支援体制、抱える社会的課題に対する夫・家族の認識も確認し、夫・家族のキャラクターを探り、必要と判断した場合にはキーパーソンへの介入も検討していた。さらに妊婦の妊娠・出産・育児に向けた思い、出産・育児に向けた準備状況、経済状況についても確認していた。また精神疾患合併や心身の不調がある妊婦については、他科との連携の必要性や自施設での分娩の可否、保健師との連携の必要性をアセスメントし、妊婦と問題意識を共有していた。妊娠期最後の面談では妊婦の変化をフィードバックしたり、分娩に気持ちが向かう援助を行っていた。
