项目摘要

本研究の目的は、インスリン療法を導入された外来通院中の妊娠糖尿病妊婦が生活の中で血糖コントロール方法を見出す過程を明らかにし、産科外来の看護職が、妊娠糖尿病と診断され自宅で血糖コントロールに取り組む妊婦に提供すべき具体的かつ効果的な看護支援モデル（案）を作成することである。これまでの研究ではインスリン療法を導入された妊婦のみを対象としたものはなく、インスリン療法を実施する妊娠糖尿病妊婦が、食事療法や運動療法のみで血糖コントロールに取り組む妊婦と同様の思いや体験をし、同じ過程を辿って生活の中で血糖コントロール方法を見出すことが可能かは明らかになっていない。インスリン療法を導入された妊婦は、インスリン療法について学び、低血糖を起こさず妊娠期の目標範囲内に血糖コントロールができるよう生活を調整すること、低血糖への対処など、特有の体験や困難さがあることが予測される。そのため、インスリン療法を導入された外来通院中の妊娠糖尿病妊婦が生活の中で血糖コントロール方法を見出す過程を明らかにするため、半構成的面接法による質的記述的研究を実施した。2022年度は６名の外来通院中のインスリン療法を実施する妊娠糖尿病妊婦に研究協力の同意を得て、インタビュー調査を実施した。インタビューは、昨年度と今年度で合わせて９名の妊婦に実施し、1名当たりのインタビュー回数は２回から３回であった。インタビューでは、妊婦の妊娠糖尿病や治療に対する理解、妊婦が自宅で実施している血糖コントロール方法、インスリン療法開始時の思いや取り組みについて聞き取った。9名の妊婦のインタビューで得られたデータは、現在分析を行っており、研究結果は次年度に学術集会並びに論文投稿により、公表予定である。さらに、本研究及び研究者がこれまで実施した研究結果より、産科外来における妊娠糖尿病妊婦の看護支援モデル（案）を作成する予定である。

The purpose of this study is to provide specific and effective nursing support to pregnant women by introducing new therapies into the life of pregnant women with gestational diabetes mellitus in general hospitals, clarifying the process of blood glucose regulation, obstetric nursing, diagnosis of gestational diabetes mellitus, and home glucose regulation. This research is aimed at introducing the concept of pregnancy therapy, diet therapy and exercise therapy for gestational diabetes mellitus, and exploring ways to improve the quality of life for pregnant women. With the introduction of INSENCE therapy, pregnant women will learn more about INSENCE therapy, develop hypoglycemia, and adjust their lives within the target range of pregnancy. It is predicted that there will be no changes in the treatment of hypoglycemia, and unique physical and psychological difficulties will arise. The study of the introduction of the treatment of gestational diabetes mellitus in the outside world and the qualitative description of the treatment process of gestational diabetes mellitus In 2022, the consent for collaborative research on gestational diabetes mellitus was obtained and the application for medical treatment was implemented in 6 foreign hospitals. The number of pregnant women in the past year has been reduced to 9, 1 and 2 respectively. In pregnancy, diabetes mellitus treatment, understanding, pregnant women's home to implement the blood glucose control method, the start of treatment, the choice of groups, the choice of information 9 pregnant women's research papers were published, analyzed and published in the next academic year. This study and other investigators have been working together to develop a plan for obstetric care support for pregnant women with diabetes.
