2022年度は、研究対象者の新たなリクルートが困難であった。その理由は、研究参加の基準が厳格であることに加え、新型コロナウイルス感染の懸念による研究参加の拒否事例があったことである。研究開始当初から上記理由のために研究参加者が予定よりも少なくなっていたが、今年度においては新型コロナウイルス感染拡大の影響を強く受けた。今後も対象者のリクルートに難渋することが予測されるため、対象者数が目標症例数である46名（各群23名）に到達していないが、本年度以降の対象者の募集を中止することとした。2023年度は、本研究のデータを解析し、研究成果の発表および論文の掲載を目標とする。この目標を達成するために、研究期間の再々延長を申請した。今年度までに研究に取り込まれた対象者は、22名（血流制限下運動群：11名［平均年齢73歳、男性2名、女性10名］、標準運動療法群：11名［平均年齢72歳、男性2名、女性10名］）であった。すべての対象者は、途中離脱することなく、術後3か月の最終評価を完了した。研究期間中において、血流制限下運動において懸念されるリスク（過度な循環応答や横紋筋融解症、深部静脈血栓症など）や、運動中における膝関節痛の増悪による運動持続が困難となるケースは観察されなかった。本研究のメインアウトカムである術直後の術側大腿四頭筋の筋力低下（術前から術後4日目における筋力低下率）に関しては、2群間に有意な差は認められなかった（血流制限下運動群［11名］：－72±11%、標準運動療法群［11名］：－75±12%）。また、術前後の術側大腿四頭筋筋力（術前、術後4日、術後1か月、術後3か月）においても、2群間に有意な差は認められなかった。本研究で採用した血流制限下運動は、TKA待機患者において安全に実施可能であることが示されたが、術後の大腿四頭筋筋力の回復に影響を及ぼさないことが示唆された。

In 2022, it will be difficult to find new research subjects. Reasons for participation in the study include the criteria for participation in the study, the suspense of new types of infection, and the refusal of participation in the study. The study participants were given a predetermined number of reasons to start the study, and this year they were strongly affected by new types of infections. In the future, there will be 46 target cases (23 in each group), and the recruitment of target cases will be suspended this year. In 2023, the analysis of the research results, the presentation of the research results and the purpose of the paper were discussed. The purpose of the study was to extend the duration of the study. This year, 22 subjects were selected (11 subjects [average age 73 years, 2 males, 10 females] in exercise group under blood flow restriction, 11 subjects [average age 72 years, 2 males, 10 females] in standard exercise therapy group). The final evaluation was completed 3 months after surgery. During the study period, exercise under blood flow restriction was difficult to maintain (excessive circulation, striated muscle lysis, deep vein thrombosis), knee pain during exercise was difficult to maintain. In this study, there was a significant difference in muscle strength of the quadriceps muscle on the operative side after surgery (muscle strength reduction rate before surgery and 4 days after surgery) between the two groups (exercise group under blood flow restriction [11]:-72 ± 11%, standard exercise therapy group [11]:-75 ± 12%). Before and after surgery, the muscle strength of the quadriceps of the thigh (before surgery, 4 days after surgery, 1 month after surgery, 3 months after surgery) was intentionally different between the two groups. This study was conducted to evaluate the effects of exercise under blood flow control on the recovery of quadriceps muscle strength after TKA.
