项目摘要

本研究の目的は，リハビリテーションの観点から日常生活動作に着目した肺炎患者のクリニカルパスを作成することである．2022年度は，臨床現場で肺炎患者を担当した経験のある作業療法士にオンラインでのインタビューを実施した結果をまとめ，その結果を専門雑誌に投稿し，修正を繰り返した．原稿をまとめるうえでは，第32回日本呼吸ケア・リハビリテーション学会学術集会に参加し，肺炎の診療に関与している医師や理学療法士，作業療法士，言語聴覚療法士などと意見交換を実施した．本研究の意義は，市中肺炎患者が過度な安静を強いられることなく，急性期病院から自宅退院が可能な患者は早期に自宅退院ができるようにするため,リハビリテーションの適切な介入時期，介入内容を提言することである．日常生活動作に着目するという点で，日常生活動作の練習を主に実施する作業療法士の意見を集約し，論文にまとめている．専門雑誌への投稿は，査読者とやり取りを行う中でより洗練された文章の構成が可能となり，多くの人の目に触れるものとして公表できるものとなる．市中肺炎患者に関わる作業療法士の意見を集約した本論文は，市中肺炎患者を担当したことのない経験の浅い作業療法士や作業療法士が関与していない施設において，日常生活動作をどのように支援すべきかを知る有効な手引きとして役立つのものとして考えられる．一方，2019年度にアンケート調査を実施した施設の中で，市中肺炎患者の取り組みが優れた施設のリハビリテーションプログラムを多施設で実施するという介入調査の可否を2022年も模索したが，新型コロナウイルスの流行が終息しつつあるとはいえ，医療現場では協力ができないとの結果となり，介入調査による提言は断念せざるを得ない状態であった．したがって，最終年度となる2023年度は，介入調査による提言をまとめるのではなく，質的調査による結果をまとめることとなる．
