本研究の目的は、訪問看護経営の持続性に影響を与えている訪問看護需要の低い予見性への課題に対応するために、疾患別の利用期間の実態を調査し、利用期間に影響を与える関連要因について地域特性を加味しながら検討することである。その前に、地域特性の概要を把握するため、公開されている2次データを用いて、全国の都道府県における市区町村ごとの訪問看護の供給状況を調査した。訪問看護の供給と各地域の人口密度や高齢化率との関連を調査したところ、訪問看護の供給状況には地域格差がみられ、訪問看護の供給と人口密度には正の相関が、高齢化率とは負の相関がみられた。これらのことから、訪問看護の供給は住民の看護ニーズよりも事業所の経営的な事情が優先される可能性が明らかになった。訪問看護における需要の低い予見性によるマネジメントへの影響について、訪問看護事業所の管理者にヒヤリング調査を実施したところ、いくつかの課題が明らかになった。まず、実践レベルにおいては、コントロールが難しい需要に対して柔軟に看護師を配置することにより対応し経営を持続できていること、ケアニーズや看護必要度による需要の把握は疾患のみではアセスメントが難しいことなどが挙げられた。これらの結果から、調査によって把握できる需要は真の訪問看護ニーズとは言い切れず、訪問看護の供給状況により大きく影響を受けており、供給が需要を生み出している可能性が考えられる。また、実践においては、柔軟な提供体制をとることが地域の訪問看護ニーズに対応するために重要であることが示された。加えて、韓国の研究者との交流により、地域・在宅における看護ニーズの把握と訪問看護の提供体制・役割については、両国において共通して注目されている課題であることが分かった。
