项目摘要

本研究は，抗がん薬を扱う病棟・外来に勤務する病院職員（医師，看護師，薬剤師，看護助手，事務職員，清掃職員）の唾液と尿のシクロホスファミド（以下，CPA）定量測定によって含有量（被ばくの実際）を分析し，各職種のガイドラインの遵守状況と認識を明らかにすることを目的とした。2020年度から開始した調査は2021年度までで合計10施設からの協力を得て医師2名，看護師33名，薬剤師12名，事務職員2名，清掃職員1名の唾液と尿のCPA分析を実施した。分析はシオノギファーマ分析センターに依頼した。結果，看護師2名，薬剤師2名の尿中からCPAが検出された。2022年は協力の得られた1施設で2名の看護師の唾液および尿中のCPA含有量の調査に加え，抗がん薬曝露状況の環境調査を実施した。2名の看護師の唾液，尿中からはCPAは検出されなかった。環境調査ではCPA（シクロホスファミド）, 5-FU（フルオラシル）, GEM（ゲムシタビン）, PTX（パクリタキセル）の汚染状況を調査した。結果，各抗がん薬が最も多く検出された場所はCPAがトイレの換気扇9.48ng，5-FUはトイレ便座足元周辺41.0ng，GEMはトイレ便座足元周辺6270ng，PTXも同様にトイレ便座足元周辺206ngであった。調査した施設では毎日，清掃員による掃除が実施されていたが，換気扇は行われていなかった。これらの結果について詳細を分析中である。
