项目摘要

新型コロナウイルスの感染拡大による研究代表者及び研究分担者、研究協力者の所属機関の所属機関の出張制限や研究活動指針等の各種制限や相手国の事情などのため、現地調査が十分に実施できなかった。その為、繰越による年度に不完全であるが実施した各種資料等を中心に解析を進めた。１．調査対象地の一つであるマレーシア、サバ州のマラリア感染状況について、サバ州Ranau地区において採集したヒト血液サンプルを検討対象とした。Ranau地区は内陸で標高1,000M、人口約10万人、その多くは農業従事者であった。調査及びサンプル解析の結果、以下の結果を得た。① Plasmodium knowlesi(以下Pk)感染者数は年々増加傾向にあった。② Pk感染者の多くは13歳以上の男性であり、農業従事者であった。③ Pk感染者は、森林周辺の農耕地で媒介蚊に刺された記憶があるため、森林周辺が感染地と推測された。④ COVID－19感染拡大による行動制限のため、マラリア患者の減少がみられた。⑤ヒトマラリア感染の患者はなく、Pk単独感染の患者だけであった。２．Ranauと同じような自然環境を持ったベトナム北部のLai Chau省を比較対象地とした。ベトナムも渡航が不可能となった直前のヒト末梢血サンプルの解析を行った。Lai Chau省では2017年以前ではマラリア感染者は0であったが、2017年、2018年に数例の三日熱マラリア患者が数名認められた。しかし、2019年サンプルを解析すると、56例の三日熱マラリア患者を見出した。特記すべき事項として、2021年のサンプルではこれまで検出されなかった熱帯熱マラリア患者1例を見出した。しかし、Pkを含む他のマラリア感染は認められなかった。以上の解析結果を得、詳細な解析に進むヒントを得、各種制限のない現地疫学調査が可能なりしだい調査を再開する。
