令和2年度より疾患レジストリを基盤としたランダム化比較試験の実践を目的とした研究活動を開始した。その妥当性検証のために、心不全領域で新たに承認された薬剤（ARNI、SGLT2阻害薬）を対象とした2X2ファクトリアルデザインの多施設ランダム化比較試験の研究計画書を作成し、令和3年7月に主研究機関となる慶應義塾大学倫理委員会の承認を得た。UMINに研究計画の登録を行なうと共に（UMIN000045229）、各参加施設での実施許可の申請、ランダム化ソフトウェア、電子的データ入力システム（EDC）の開発を行った。また、臨床研究コーディネーター（CRC）の業務委託など の本研究の実施に向けた準備を進めた。 同年12月には参加施設に向けた説明会を複数回開催した。令和4年1月から慶應義塾大学病院での患者の登録を、2月より杏林大学病院、埼玉医科大学附属病院、東京慈恵会医科大学附属病院、3月より国立埼玉病院での登録を開始した。なお令和4年4月より、米国で多数のランダム化比較試験を実施しているJohn Spertus教授の研究室での活動を開始し、その方法論やデータ取得に関して直接的な指導を受けた。また同研究室が得意とする循環器内科領域における患者報告アウトカム（PRO）の本研究への応用に関しても定期的な議論を行い、心不全に疾患得意的なPROであるKCCQを本研究のエンドポイントとして扱うこととした。渡米後も研究事務局と定期的に密な連絡をとりながら、順次実施許可申請及び臨床研究コーディネーターの業務委託契約を終えた共同研究機関での患者登録を開始し、令和4年度末時点で合計13施設で患者登録を行い、目標症例数の約８割の登録を終えた（212/240例）。令和5年6月頃に登録患者数が、目標症例数に達する予定であり、そこから半年のフォローアップ期間を経て、年度末には最終的な統計解析まで終える予定である。

In April 2004, most of the comparative experiments were carried out in the laboratory of Professor John Spertus, and the activities of the methodology were directly guided. In the same laboratory, patients reported a loss of PRO in the field of circulatory medicine, and this study was used in connection with regular discussions and KCCQ in the field of cardiac insufficiency. After the transition, the Research Affairs Bureau conducted regular and close contact with each other, and the license application and clinical research commission contract were completed in sequence. The patient registration of the joint research institution was started, ordered, and completed at the end of the fourth year. The total number of patient registrations was 13, and the number of target cases was about 8 (212/240). The number of registered patients and the number of target cases reached a predetermined time in June 2015, and the final statistical analysis reached a predetermined time in June 2015.
