项目摘要

在宅生活を継続する認知症の人と家族介護者は、これまで異なる場で支援が行われてきた経過がある。認知症の人については診断後の医師や相談員による医療的支援に加え、認知症当事者同士のピアサポートが行われている。家族介護者は1980年に発足した認知症の人の家族の会による介護者同士のピアサポート活動や各市町村自治体が地域支援事業（任意事業）などで実施される介護者交流会などが行われてきた。認知症ケアによって最も課題となるのBPSDは、介護者や周囲との関係悪化や環境への不適応により出現することから、在宅生活における認知症の人と家族の関係性へのアプローチは重要である。しかし、前述の通りこれまで両者それぞれへのアプローチが中心であり関係性を支援するアプローチが不足していた。そのことから本研究では、すでにオランダ等で展開されている、統合ケアプログラムの日本への適応と開発を目指している。1年目である2020年度は、諸外国における認知症の人と家族の統合ケアプログラム「ミーティングセンターサポートプログラム」の文献調査よりプログラムのモデル、構造、そして運用や既存のサービスからの転換など多くの示唆を得ることができた。2年目の2021年度は、次の2つの調査を行った。①在宅介護サービスに携わる従事者を対象に、フォーカスグループインタービューにより、統合ケアプログラムの適用方法や可能性を検証した。②全国18ヶ所の認知症カフェに参加する家族を対象に訪問理由やニーズに関する質問紙調査を実施し、認知症カフェの効果モデルの確認を行った。3年目は、2年目で実施した認知症カフェの調査の詳細分析を行い評価尺度の開発を試みた。同時に、フォーカスグループインタビューのテキスト分析に着手した。

Family caregivers for people with cognitive disorders who live in the home support them in different fields. People with cognitive disorders may receive medical support from doctors and counselors after diagnosis, or from colleagues with cognitive disorders.
