项目摘要

自閉スペクトラム症児（以下、ASD児）への効果的な支援法として、日常環境発達行動支援法(Naturalistic Developmental Behavioral Intervention: NDBI)がある。それは、発達カリキュラムをもとに子どもの言葉や動作などの発達を評価し、共同注意や模倣といった発達の軸になるような行動を標的とし、複数の日常場面に学習機会を設定し、学習を進めていく方法である。本年度の研究では、以下の3つの研究を行った：研究1. レビュー研究、研究2. 探索的介入研究、研究3. 介入研究。研究1では、NDBIに関するシステマティックレビューを行い、既存のエビデンスを整理し、各研究で介入の効果をあげた条件を明らかにした。その結果、NDBIは日常生活での支援を重視し、保護者が介入技法を学んで家庭で実践することが主となることが分かった。また、NDBIは自由遊びにおいてASD児の言語発達に大きな効果があるが、重度の知的障害を伴う児や言語発達に顕著な遅れが見られる児に対しては、介入効果が比較的低いことが明らかとなった。研究2では、研究1の結果を基に、NDBIのオンライン介入研究を実施した。具体的には、重度のASD児2名とその保護者を対象に、約10週間のオンライン介入を行った。その結果、対象児2名のうち1名において、「笑顔」と「目線」の有意な増加がみられた。また、対象児2名においても、「発声発話」の増加がみられたが、変動性が大きかった。このことから、研究3での実施に向けて研究手続きを調整した。研究3では、対面支援とオンライン支援に分けて、自閉症児とその保護者１０名を対象にNDBIを実施し、その効果を検討した。標的行動の変動性を減らすため、実験デザインや包括的な事前評価の実施など、研究手続きの調整を行った。
