项目摘要

小規模多機能居宅介護事業所での調査を基に看取りケアにかかわるスタッフ育成に不可欠なことについて検討を行った。看取りケア教育で経験の浅い介護者の不安を和らげる役割を担う者が必要であるが、その役割をマニュアル化することはできない。また看取りケアでは介護者の感度が要求され、そのために大切なのは対象と介護者の継続的な関係である。マニュアル化できない理由として、看取りケアがプロセス・コンサルテーションであるためである。看取りケアは点でなく線であり、介護者はその経過を対象と共に過ごす。その相互応答の積み重ねが、介護者の感度を磨き、看取りにおいて、対象が良い死に方ができたと介護者が思えることにつながる。シャイン,E(2009)は、支援者の役割として基本的に①情報やサービスを提供する専門家、②診断して、処方箋を出す医師、③公平な関係を築き、どんな支援が必要か明らかにするプロセス・コンサルタント、の3種類あるという。対象と介護者の継続的で相互的な関係では、介護者はプロセス・コンサルタントである。この支援は個別の関係が規定し、支援の内容は標準化が困難である。但しプロセス・コンサルタントとしての姿勢は獲得可能なスキルといえる。2022年は住宅型有料老人ホームで看取りケア教育の状況調査を実施した。調査対象施設は生活保護受給者が8割の施設で貧困単身高齢者が多く入所している。た。現場ではCOVID-19の影響を強く受けており混乱が続いている状況にあるが、貧困単身高齢者を支援するケアスタッフの看取りの現状に関する知見を得た。
