项目摘要

たんの吸引と経管栄養は、所定の研修を受けた介護職が「医療的ケア」として行うことができるが、医行為であるために他のケアよりも事故の危険性が高いとされている。特に在宅の医療的ケアでは、それぞれの家庭環境に合わせた配慮が必要になる。本研究では、在宅の医療的ケアにおける事故の実態把握をもとに、介護現場に適した事故防止策を構築することをめざしている。2022年度は、３つの訪問介護事業所に勤務し医療的ケアを行っている介護職にインタビュー調査を行った。事故についてだけでなく、在宅で医療的ケアを行う意義も確認した。対象となった介護職は、事故を起こした経験はしていなかったが、気管カニューレが抜けかかっていた、吸引時の鼻出血、などのヒヤリハットの経験をしていた。「医療的ケア」には業務範囲の制限があるため、気管カニューレのカフ圧が低くても調整できないこと、アンビューバッグを日常的に使えないことなどに、支障を感じるということだった。事故を防止するために喀痰吸引の深さや吸引圧などには規定・指示はあるものの、実際には規定では痰が引けないこともありグレーゾーンがあるとのことであった。事故防止策として、記録の電子化により引継ぎや情報共有を即時に行っていること、いつでも相談できる体制を整えていること、介護職の個別性に合わせて研修期間を調整していること、手順書を共有し業務前に確認していること、などがあげられた。在宅で医療的ケアを行う意義として、施設のような集団ケアではなく一人ひとりのペースに合わせてケアができる、利用者の生活に寄り添える、高齢者も障がい者も対象になりキャリアの幅が広がる、などがあげられた。2023年度は残りの調査を行い、最終的な分析と論文作成を行う予定である。
