全身ルチーンCTによるがん検診効果のシステマティックレビュー（SR）については外傷患者の全身CT研究を対象に研究計画簡易版を国際レジストリPROSPEROに登録、Medicine誌に2021年1月に掲載されたプロトコールに従いSRを実施中である。適格基準に該当する採用研究27件中、定義に基づく全身CTを統一して実施した研究は5件のみで、体幹部のみ（頭頚部を除く）が3件、他は多様な撮影部位（頭部、胸部、腹部等の最低1部位以上の組み合わせ）が合同的に評価された。また造影剤を含む撮影プロトコールにも異質性が高く、外的妥当性に懸念が認められた。一方、外傷以外の偶発所見の臨床的重要性・重症度の操作的基準と経過観察方法、さらには解析単位にも研究間で高い異質性があった。2022年3月に文献アップデートを実施し、2022年度最終解析に入る予定である。CT検査プロトコールに造影剤を追加的に使用する害についてのSRでは16件の非ランダム化比較研究（NRS）を同定し、過去に造影剤副作用歴がある場合の対応法とその比較効果についてメタ解析を実施した。6件のNRS（7155人が参加）では副作用を起こした可能性がある造影剤を変更するのみで約6割（リスク比=0.39; 95%信頼区間: 0.24-0.58)の副作用予防効果が示唆され、これは先行副作用の重症度や予防投与（ステロイド剤、抗ヒスタミン剤、両者併用）の有無に拠らず安定していた。本結果はRadiology誌に投稿して改訂中、近日中に受理となる見込みである。予防投薬の比較効果については現在最終解析中であり、結果を近日中に投稿予定である。後方視的一次研究については上記の複雑な偶発所見の操作的基準を標準化し、データ入力を行うアプリの開発を行った。これに基づき偶発所見についてデータ抽出を行い、本邦3次医療機関からの結果を纏める予定である。
