项目摘要

[目的] 動的肺過膨張（DLH）は慢性閉塞性肺疾患（COPD）における呼吸困難および運動耐容能低下の重要な要因である．気管支拡張薬はDLHを軽減させるが，呼吸リハビリテーション（PR）のDLHに対する効果は不明である．本研究の目的は外来通院PRがDLHを改善させるかどうかを検討することである．[方法] 外来通院中の安定期にあるCOPD患者15例（男性14例、女性1例、平均年齢78±6（SD）歳、病期Ⅰ/Ⅱ/Ⅲ/Ⅳ:1/8/3/3）を対象とした。PR前にスパイロメトリー、6分間歩行試験と過呼吸法によるDLHおよび疾患特異的健康関連QOLであるSGRQ-Cを測定した後、1～2週間に1回外来受診し、PR（ストレッチ、上下肢筋力強化運動、持久力トレーニング、呼吸法指示、自主トレーニング指導）をおこない、3か月後と6か月後に評価をおこなった。本研究は信州大学医倫理審査委員会（承認番号4373）および市立大町総合病院倫理審査（承認番号11）で承認を得ている。[結果] PR3か月後では肺活量、1秒量、6分間歩行距離には有意な改善を認めなかったが、30回/分および40回/分の過呼吸後の最大吸気量（IC）は1.50±0.58L⇒1.66±0.60L, 1.42±0.54L⇒ 1.50±0.57Lと有意な改善を認めた。過呼吸に伴うICの減少量には有意な改善は認めなかった。SGRQ-Cは有意な改善が得られた。6ヶ月後の評価が出来たのは9例であり改善傾向を示す者の統計的検討は出来ていない。[結論] COPDに対する呼吸リハビリテーションは過呼吸時の肺過膨張を軽減させるが過呼吸に伴うエアートラッピングに対する改善効果は見られないと考えられた。

[Objective] Dynamic hyperinflation of lung (DLH) is an important cause of dyspnea and exercise tolerance in chronic pulmonary disease (COPD). The results of DLH are unknown. The purpose of this study is to improve the PR of DLH. [Methods] 15 COPD patients (male 14, female 1, mean age 78±6 (SD) years, stage Ⅰ/Ⅱ/Ⅲ/Ⅳ:1/8/3) were recruited from the hospital. Before PR, 6-minute interval test, DLH test and disease-specific health related QOL test, SGRQ-C test, 1 visit every 1 to 2 weeks, PR test, upper and lower limb muscle strengthening exercise, endurance test, respiratory method instruction, autonomous test guidance), 3 months later and 6 months later. This study was approved by the Medical Ethics Review Committee of Shinshu University (Approval No. 4373) and the Ethics Review Committee of Municipal Omachi General Hospital (Approval No. 11). [Results] PR3 months later, lung capacity, 1 second volume, 6 minutes walking distance, intentional improvement, 30 cycles/min, 40 cycles/min, and maximum intake capacity (IC) after breathing were 1.50±0.58L <$1.66 ±0.60L, 1.42±0.54L <$1.50 ±0.57L, intentional improvement. The amount of IC that has been reduced by breathing is intentionally improved. SGRQ-C is intended to improve. 6 months later, the evaluation results showed that 9 cases were improved, and the statistical results showed that 9 cases were improved. [Conclusion] COPD is associated with respiratory distress and pulmonary hyperinflation.
