本研究は、心臓血管外科手術手技における、運針中の注視視点について定量的に可視化することで、熟練指導医と修練医が行う血管吻合の違いを解析することを目的としています。また、この解析結果をふまえて運針技術の上達効果について最終評価を行うものです。これまで、当初の研究計画にあったように小口径の血管に対する細かい運針における注視視点解析は、測定技術上困難と判断されたことから、研究計画の見直しを行なってきました。見直しの変更点としては、視点追跡の対象となる冠動脈吻合中の運針については、吻合中の手術動画を大画面に映し出し、この画面上の運針の動きをアイトラッカー装置（注視追跡用ゴーグルとデータ計測専用スマートフォンから構成）を用いて計測する方法です。この計測方法によって、大まかな視点追跡のデータが収集可能となりました。次に、計測方法が確立した段階で、直径約２ｃｍほどの人工血管どおしを吻合して、リアルタイムに吻合中の視点追跡データを収集することを当該年度は行いました。これによって、手術経験年数の異なる医師による視点追跡の特徴に違いのあることが分かってきました。研究計画の遅れにより、令和５年度まで研究期間の延長をすることになりましたが、本年度はデータサンプル数の拡大と解析を行ない、最終報告ができるよう研究を遂行する計画です。現在はデータ解析と発表、論文化を今年度に行えるように研究を遂行中です。

The purpose of this study is to analyze the surgical technique of cardiovascular surgery, the observation point of view, the quantitative visualization, the skilled guidance of doctors, the practice of vascular anastomosis and the violation of vascular anastomosis. The results of this analysis are discussed in detail below. The original research plan is difficult to analyze and determine the technical difficulties of small diameter blood vessels. The method for measuring the movement of the needle in the coronary artery anastomosis is described in the following: the movement of the needle in the coronary artery anastomosis is reflected on a large screen, and the movement of the needle on the screen is detected by a device (composed of a measurement device for tracking the movement of the needle). This measurement method is based on a large number of observations. In addition, the measurement method has been established, and the artificial blood vessel with a diameter of about 2 cm has been matched. The viewpoint tracking data in the match has been collected. The number of years of operation varies from doctor to doctor. The study plan is to be completed, ordered, and extended for 5 years. The study plan is to be implemented in the final report. Now, we're going to analyze and discuss culture, and we're going to do it.
