项目摘要

健康志向には不安感など主観的心理社会評価が関連する。しかし、経時的に変動する社会構造による健康志向への影響が不明である。現代社会は、所得格差や社会保障制度の不安定化などを特徴とする不確実性社会という社会構造と言える。本研究は、健康志向に関わる不確実性社会という社会構造の横断的・経時的影響を解明することを目的とする。まず経時的な不確実性社会指標データセットを作成する。つぎに、宮城県民対象の社会調査を実施する。申請者が2011年に実施した宮城県民社会調査データと結合し、2時点データセットを作成する。最後に、不確実性社会指標と2時点データセットを統合したデータセットを横断的・経時的に解析する予定であった。19年度はまず、マクロ水準における不確実性社会指標データセットの作成をおこなうために、不確実性社会を示す指標作成のため、文献レビューをおこなうこととした。不確実性社会という社会構造として、高齢化率、失業率、医療提供体制・社会保障制度の変容を指標として想定し、これらに関する文献収集をおこなった。高齢化率、失業率は不確実性社会を示す指標として有効であることが、文献をレビューしたところ明らかにされた。また、医療提供体制・社会保障制度についても、文献レビューをしたところ、不確実性を示す指標として充分に妥当であると考えられた。この、医療提供体制・社会保障制度については、さらに、不確実性社会の指標としての妥当性を検討するために、公的公開データを使った二次データ分析をおこなった。国の予算に占める社会保障費の割合が経時的にどのように変化しているかということと、国民の健康との関連について、現在のところ、分析中である。レビューに基づき20年度は宮城県の市区町村及び二次医療圏単位の官公庁公開データからデータセットの収集に努めた。また、分担研究者と共同で宮城県対象の社会調査の調査票設計を予定していた。
