项目摘要

2022年度は、2021年度に引き続き、新型コロナ感染症の流行とその対策により、現地研究活動に制約、支障があり、現地における研究活動のいくつかが実施できなかった。そこで、現地で実施可能な調査を進めた。本研究対象地域である、ケニア西部ホマベイカウテンィのビタ・サウス及びンディワサブカウテンィにおいて、地域住民や医療従事者によるスナノミ症に対する治療実践の実態や関連する知識を明らかにするため、量的及び質的データを収集し検討する混合研究法を実施した。量的データではスナノミ症罹患歴がある18歳以上の421名を対象として、各自が実践した治療方法を調査した。質的調査では、60歳以上の高齢者20名、医療従事者（クリニカルオフィサー及び看護師）10名に対し、治療方法、その治療法を採用した理由を聴取した。量的調査では、研究参加者の45％が国家スナノミ症対策ガイドラインに記載されている治療法を採用していたが、それ以外は、不適切な自己摘出など、ガイドラインにない治療を実践していた。それには、知識不足や診療施設に受診しないことなどが関連していた。60歳以上の高齢者への聴き取りでも自己摘出が多く、さらに医療従事者による治療法も外科的摘出術が多かった。ガイドラインの治療を選択しない理由として、地域住民では他人からの示唆、医療従事者ではスナノミ症に対する医薬品の供給不足が挙げられた。本研究結果は、有効かつ安全で持続可能な治療法検討の基礎データになるとともに、地域住民に対する啓発活動、医療従事者に対するスナノミ症診療の関する研修の重要性を示した。比較対照研究に関しては、現地で購入・適用、継続がより容易な治療法の有効性を検討する研究として、ケニアでの倫理審査承認を得られるようになった。
