项目摘要

2021年に実施した看護師5名へのインタビュー結果を基に、外国人患者に対するケアを提供する際に「困難を感じた場面とその工夫」、「困難を解決するための学習ニーズ」について、Riessmanのテーマ的ナラティブ分析を参考に質的探索的に「何が語られたか」に関する共通のテーマを抽出した。困難と感じたケア場面では＜死生観を含む価値観の違い＞、＜治療・検査に関連する困難＞、＜日常的なケアに関する困難＞、＜病院のルールに関連した困難＞、＜コミュニケーションに関する困難＞、＜病院・組織全体の管理に関する困難＞の6つの困難に関するテーマが抽出された。工夫したことについては＜宗教に関連した配慮＞、＜翻訳機器や医療通訳、ビジュアルエイド等の活用＞、＜専門的な多職種協働＞が抽出された。この分析結果は、先行研究とほぼ一致した。この結果は2022年7月に開催された第24回日本医療マネジメント学会学術総会で発表した。逐語録から学習ニーズについては、＜外国人患者の日本の医療に対する認識と期待の把握＞、＜外国人患者の文化・生活様式と価値観の把握＞、＜外国人患者と相互理解を深めるためのコミュニケーションスキル＞、＜外国人患者に対応する多職種協働の在り方＞の4つのテーマが抽出された。外国人患者が日本の医療をどのように認識し何を期待しているかという意識の把握や、受け入れる際に配慮が必要な、外国人患者の宗教や文化等に対する知識獲得のニーズが把握された。この知識を前提にコミュニケーション上の齟齬を未然に防ぐための対策や倫理的問題の対応、外国人患者と相互理解を深めるためのコミュニケーションスキルの獲得に関するニーズがあった。さらに多職種の役割分担に関する学習ニーズが明確となった。この分析結果は2022年12月に開催された第42回日本看護科学学会学術集会にて発表した。
