本研究の目的は、医療者による誤った行為が発生したとしても、患者に誤った行為が実施されるまでの間に、医療事故が未然防止できる人材を育成することである。2022年度は、日本医療機能評価機構における報告事例の分析と手術室看護師に対してのインタビュー調査を実施した。日本医療機能評価機構の医療事故情報収集等事業における2021年度に報告されたヒヤリ・ハット報告事例6253件の内、「手術室」で発生し「看護師」が当事者となった報告事例は328件(5％)であった。328件の内「医療機器等」に関する報告事例は217件(66％)であった。報告事例の内容としては、医療機器の破損、紛失などであった。「医療機器等」に関する報告事例217件の内、医療事故が未然防止された事例は、15件(7%)であった。15件の内容は、医療機器の準備段階で破損や異物の付着に気付き、患者に使用する前に回避されるなどであった。インタビュー調査の結果、手術室における医療事故の未然防止として「医療機器」と「薬剤」に関する内容が得られた。インタビュー調査によって得られた医療機器に関する事例は、準備段階で点検を行っていたにも関わらず、劣化などの何かしらの原因によって手術中に破損し、患者に使用する前に発見された内容であった。日本医療機能評価機構の医療事故情報収集等事業におけるヒヤリ・ハット報告事例においても同様の事例が報告されており、手術室における医療機器管理の難しさが挙げられる。手術室においてより安全な医療・看護が提供できるよう医療事故の未然防止のみでなく、医療機器管理の安全使用に向けた対策が必要であると考える。

The purpose of this study is to develop the wrong behavior of medical personnel, prevent the wrong behavior of patients, and cultivate talents. In 2022, the Japanese Medical Function Evaluation Agency conducted an analysis of the cases reported and a survey of the operating room nurses. In the 2021 annual report of the Japan Medical Function Evaluation Organization's Medical Accident Information Collection, there were 6253 cases reported, and 328 cases (5%) of "operating rooms" and "nurses" were reported. Among the 328 reported cases, 217 (66%) were related to medical equipment.
