项目摘要

2019年度～2020年度は口腔粘膜炎リスク要因のスクリーニング指標作成の基礎データを得るために、地域がん診療拠点病院に指定されている900床規模の大学病院において、診療録（50例）を基に後ろ向き調査を行った。2021年度は、がん化学療法を受けた患者のうち以下の条件を満たす患者を対象に前向き調査を行った。厚生労働省の「重篤副作用疾患別対応マニュアル」をもとにリスク要因として、口腔衛生状態、免疫能、栄養状態の3点を調査した。口腔衛生状態は口腔乾燥、舌苔、歯磨き・含嗽回数、免疫能は、白血球数、好中球数、栄養状態は、血清アルブミン値、総タンパクを調査した。対象の治療レジメンは、Ara-C療法、MTX療法、R-CHOP療法、CE療法、AC療法、FOLFOX6療法、FOLFIRI療法など16種類であった。有害事象共通用語基準：CTCAE Ver5.0のGrade分類で口腔粘膜炎の評価Grade0-1を1群とし、Grade2以上を2群（口腔粘膜炎発症群）として比較した。1群は45例（64.3％)、2群は25（35.7％)、であった。口腔粘膜炎発症に関連する要因として、口腔衛生状態は、Grade2以上の口腔乾燥、Grade3以上の白血球減少、Grade3以上の好中球減少が抽出された。口腔衛生状態の舌苔、歯磨き・含嗽回数と栄養状態の指標とした血清アルブミン値は、両群で差はなかった。2022年度はがん化学療法を受ける患者を対象に、口腔粘膜炎リスク要因の中で、口腔粘膜炎の重症度に影響を与える要因を調査・検討し、重症度に関連する要因は、口腔衛生状態の中で、義歯・歯周炎・義歯不適合、歯磨き回数、Grade3以上の白血球減少であることを明らかにした。その結果を基に、口腔粘膜炎予防のセルフマネジメントプログラムに使用する「がん治療中の口腔ケアガイド」のコンテンツを作成した。
