本研究の目的は、外来において化学療法を受けるがん患者の緩和ケア主体の治療へのTransitional Careを実践している専門看護師、医師、訪問看護師の実践内容を明らかにし、既存文献と統合して外来における多職種・他部門との連携・協働を基盤とした「がん患者の緩和ケア主体の治療へのTransitional Care Model」を開発することである。2020年度は、外来において化学療法を受けるがん患者の緩和ケア主体の治療へのTransitional Careを実践するがん看護専門看護師の実践に関する面接調査の準備、実施および分析を目標に取り組んだ。インタビューガイド作成のために文献検討を深め、緩和ケア主体の治療へのTransitional Careについて、「外来で化学療法を受けるがん患者が回復を目指す治療が難しくなることを予測し、がんに向けられた治療から今後の生活の質に向けて目標を見出し、自分にとって望ましい治療やケアを意思決定できるように、多職種・他部門との連携・協働により支援する継続的な取り組み」と定義した。所属施設の研究倫理審査を受審し承認を得た後、研究協力への依頼を行い、調査の準備中である。本研究課題は、2019年度から4年間で実施予定であったが、研究代表者の退職に伴い2020年度で補助事業を廃止させていただいた。化学療法の進歩により予後が短い時期にあっても病状や症状を抑えるために外来で化学療法などの治療が行われる場合が増えており、進行がん患者が緩和ケア主体の治療へシームレスに移行できるように支援するために必要な研究であるため、今後も継続していく予定である。
