本研究の目的は、血液・腫瘍疾患を持つ青年のSDM（Shared Decision Making）を支援する高度実践看護師への教育プログラムを開発することである。2021年度は、青年のSDMを支援する高度実践看護師の実践技能能力について、先行研究で作成した看護介入プログラムや小児看護専門看護師が実際に行っている実践と比較しながら特定できるよう検討を継続した。高度実践看護師のための教育プログラムとなるよう、授業科目の検討などを行った。青年期は、進学、就業、親子関係の変化など特有の発達に応じた新たな課題や病気に関連した課題など、多重で多様な問題に向き合い、親や医療者との相互作用のなか意思決定に取り組んでいると考える。そのため高度実践看護師は、意思決定の支援の視点を多面的にもち、理論に基づき体系的に実践する能力を養うことが求められていると考え、授業科目に取り入れる関連する理論についての検討を行った。また家族に焦点を当てた青年のSDMの既存研究などからも検討した。親は子どもには意思決定に参加する権利があると捉え、子どもが行う健康管理に一緒に責任を持つなど、決定に積極的にかかわっていることが報告されていた。先行研究からも、青年のSDMにおける「家族」への実践能力を養うことができる授業科目を取り入れることも必要であると考えられた。今後は小児看護専門看護師に意見を聴き、実際の実践と比較しながら改善点などを出してもらい、高度実践看護師のニーズに合った実践に則した教育プログラム案の完成と試行に向けて取り組んでいく。
