项目摘要

本研究は、臨床現場(静岡県立こども病院看護部)との協働研究である。これまで、新型コロナウイルス(COVID19)感染症拡大の影響を受け、臨床とともに研究を遂行することに大幅な滞りが生じ、計画修正を余儀なく強いられ遅延している。令和4年度は令和3年度の実績に伴い継続課題となった倫理的能力を促進するツールの開発(ファシリテータ人材育成、看護倫理を語る文化の育成)に取り組んだ。静岡県立こども病院における本研究の協力病棟(ICU病棟・北4病棟)である病棟管理者およびファシリテータを担う看護師と定期的な検討会議を開催し、2回/月の看護倫理カンファレンスに参加し、看護師の倫理的能力を支援した。看護倫理カンファレンス終了後は、ファシリテータする上での困難と発見についてdiscussionを行った。ファシリテータ役割には、1拡散:事例を共有し参加者の意見を聴く、2集約:参加者の意見を共有し、看護倫理上の問題を焦点化する、3拡散:対応策について参加者の意見を聴く、4集約:対応策の方向性を検討するプロセスがあるが、「2集約」の場面のファシリテータ能力の強化が重要課題となり継続的支援が必須であった。「看護倫理を語る文化の醸成」においては、カンファレンスにおける環境は重要であり、お互いの顔が見える距離で話ができる環境に整えること、自由な話ができる約束事を明確化すること、参加看護師の集中力が継続できるようにカンファレンス中のナースコール対応について検討し、カンファレンスにおける環境改善を図った。
