本研究では、産婦及び胎児にとって安全な分娩環境を提供するため、硬膜外麻酔分娩について豊富な知識と経験をもつ助産師の臨床推論の過程を認知課題分析法を用いて明らかにし、硬膜外麻酔分娩時の助産師の臨床推論モデルを構築することを目的としている。硬膜外麻酔分娩を取り扱っている4か所の分娩施設に勤務し、硬膜外麻酔分娩時の助産ケアの実践経験が豊富な助産師8名と、実践経験が浅い助産師4名を対象として、Applied Cognitive Task Analysisの手法を用いて実施したインタビュー調査の結果の分析を行った。分析にあたっては、インタビュー調査の際に使用したアニメーションによるシミュレーションモデル動画を用いたことを前提とした。シミュレーション動画の中で、産婦が分娩施設に入院してから分娩までの状況を提示し、場面ごとにその時点での、情報収集、仮説の形成、再度の情報収集、仮説の再検証という一連の過程と緊急性の判断を問うており、分析の視点とした。取得された音声データをもとに逐語録を作成し、シミュレーション動画の場面と比較し分析を行った。分析にあたっては、経験豊富な助産師と経験が浅い助産師のデータを比較検討することで、臨床推論のプロセスがより明確化されると考え、その点を含めて実施した。また、当初予定していた海外での調査については、実施することができなかったため、代替として硬膜外麻酔分娩時の助産ケアやガイドライン等の収集を行い、国内で取得したインタビューデータの分析に活用した。
