项目摘要

特定妊婦の抱える問題と訪問する助産師の支援・課題を明らかにすることを目的に、A市で特定妊婦を訪問する助産師10名に半構成的面接を行い、若年特定妊婦及び精神障害をもつ特定妊婦についてSCAT（Steps for Coding and Theorization）の手法を用いて分析した。若年特定妊婦に関する問題と支援や課題については、論文作成し掲載された。内容は以下である。助産師の語りから若年特定妊婦事例14例を抽出した。若年特定妊婦は、慣習面、金銭面、健康面、成育面などの生活基盤が不安定で、育児に向き合う姿勢の弱さ、育児技術のなさ、育児を行う家族間の関係性の不安定さがみられた。助産師は、行政等と連携を取り、育児に協力可能な家族間の調整をしながら、若年特定妊婦に包容的にかかわり、育児指導を行い、子どもとの愛着形成を目指し支援していた。助産師は、若年特定妊婦の個別性を見極め、必要と考えられる連携的支援、指導的支援、愛着育成的支援、包容的支援、育児者間関係調整支援を実施し、安定的育児関係構築へと導いていた。次に、精神障害をもつ特定妊婦については論文作成しているが掲載に時間を要している。事例は12例抽出した。精神障害をもつ特定妊婦は、自立した受診行動が取れないことで起こる心身の不安定、対人関係の困難さ、精神状況や知的能力の程度により起こる育児技術習得の困難さ、養育環境調整の困難さ、子どもへの愛着形成が進まない状況などがみられた。訪問助産師は、包容的な態度で接し、病院受診の同行、自宅での母体管理、理解力の考慮と心身の負担軽減となる育児技術習得への支援を行っていた。また、入院中の特定妊婦の問題、支援、課題について、病院勤務助産師を対象に調査を追加し、インタビューを実施し分析し論文作成中である。来年度には論文投稿する予定でる。
