本研究の目的は，医療機関での疾患の治療が終了したのち，認知症高齢者ができるだけ早期に住み慣れた自宅に退院し生活を継続するために，医療機関での退院支援のみでは困難な認知症高齢者の在宅復帰を入院直後から、所属機関や立場の異なる病棟看護師と訪問看護師が連携・協働を図るための「認知症高齢者の在宅復帰支援パス」を開発することである。2020年度は在宅復帰を担う役割を持つ回復期リハビリテーション病棟看護師，地域包括ケア病棟看護師および在宅での生活基盤を支える訪問看護師に対して、認知症高齢者への退院支援の実際についてインタビュー調査を行った。双方ともより密接な連携は必要であるとの認識はしているが，現状としては，思うような情報交換ができていないことが明らかとなった。2021年度は，回復期リハビリテーション病棟や地域包括ケア病棟での有効な退院支援のためには病棟看護師と訪問看護師との連携・協働は欠かすことができないため，認知症高齢者の在宅復帰に向け，より有効で密接な関係を築くための資料として質的分析の結果から「認知症高齢者の在宅復帰を目指すための看看連携に関する病棟看護師の評価指標」を試みた。2022年度は，前年度に作成した「認知症高齢者の在宅復帰を目指すための看看連携に関する病棟看護師の評価指標」を医療機関に勤務する認知症看護認定看護師を対象に信頼性と妥当性の検証を行った。その結果を「認知症高齢者の在宅復帰支援パス」の基礎資料とし，4つのフェーズに分類したパスを作成した。今後は，作成したパスを医療機関で活用し，その効果を検証していく予定である。

The results of the qualitative analysis of the relationship between the two groups were tested by the evaluation index of the patient's home visit and the relationship between the two groups. In 2022, the previous year, we created the index of evaluation of nursing staff for cognitive patients in the home, and the index of evaluation of nursing staff for cognitive patients in the medical service. As a result, the basic data of "home rehabilitation support for people with cognitive disorders" was created. In the future, the medical institutions will make use of this information.
