项目摘要

本研究の目的は、「よいときは記憶や了解もよく日付や病院名も答えられるが、悪いときには話がまったく通じず、状況や周辺環境の理解もできなくなる」というレビー小体型認知症(DLB)の中核的特徴である認知機能変動という症状へのケア方法を明らかにすることである。本研究では、認知機能変動の意図的な観察を通してDLBの認知機能変動を把握する方法を確立し、病院や地域の場で、DLB本人・家族・専門職者が認知機能変動を共通認識し、適時・適切な治療・ケアにつなげるツールの開発を第一の課題としている。現在のところ「認知機能変動観察指標」を作成しDLBの認知機能変動について、4つの観察項目と9段階の指標を用いて、その人の認知機能変動のタイプの特徴を示すことができた。今年度の課題として、本研究で作成した「認知機能変動観察指標」」を幅広く活用できるよう改訂することである。現在の「認知機能変動観察指標」は認知症専門病棟に入院中のレビー小体型認知症患者を対象に看護師が使用した結果から作成している。よって今年度は、この「認知機能変動観察指標」を地域で生活されるDLBの人を介護する家族介護者に使用してもらい、課題等を抽出し修正を加えることを目的に取り組んだ。20名を目標に取り組んだが現在のところ6名に留まっている。データ収集が困難な状況はこの先も大きく改善することは望めないことから、新たに次の方法で調査すること追加する。認知症看護認定看護師と老人看護専門看護師を対象に「認知機能変動観察指標」を専門職以外でも簡便に使用できる指標となるようフォーカスグループインタービューを実施することとした。この2方向の調査結果から、現在の「認知機能変動観察指標」をより洗練させ、DLBの人の生活の中でこの指標を使って、よりよい生活、最適な支援につながるケア方法の構築を目指す。

The purpose of this study is to understand the characteristics of the nucleus of small body cognitive disorder (DLB), and to clarify the cognitive function, symptoms and methods of loss. This study aims to establish a method for understanding cognitive function changes in DLB, and to establish a common understanding of cognitive function changes among DLB individuals, families, and professionals in different areas of the hospital, as well as timely and appropriate treatment. Now, the "Cognitive Function Change Index" is created, and DLB's cognitive function change index, 4 observation items, 9-level index, and all people's cognitive function change characteristics are displayed. This year's topic, this study developed a "cognitive function change index" to be used widely. The Cognitive Function Shift Index is now used by caregivers in patients with cognitive disorders and in patients with microsomia. This year, the "cognitive function change index" is selected from the group of people who live in DLB, family caregivers, users, topics, etc. 20 names are selected from the group and 6 names are reserved. The first step is to improve the quality of the product. The second step is to investigate. Cognitive Care Accreditation and Geriatric Care Caregivers: Cognitive Function Change Assessment Indicators are easy to use outside the home. The results of this two-way survey show that the current "cognitive function change index" is a guide to the construction of methods for improving, improving and optimizing DLB people's lives.
