项目摘要

本年度は介護老人福祉施設及び介護老人保健施設の利用者家族における、スピリチュアルケアや宗教的な関わりの評価やニーズ、それらに関連する要因について検討を行った。対象は、介護老人福祉施設または介護老人保健施設を利用し、亡くなってから6ヶ月以上3年未満の利用者遺族とした。回答者の利用者との続柄は、実親、義理親、配偶者、きょうだいに限定し、かつ利用者との面会あるいは電話でのやり取りが週 1 回以上あった者を対象にオンラインによる質問紙調査を実施した（介護老人福祉施設の利用者家族350名、介護老人保健施設の利用者家族350名）。結果、施設環境に対する満足度には、介護老人福祉施設では利用前の関係性、施設での葬儀、基本的ケア、スピリチュアリティの表出を支えるケアが、介護老人保健施設では基本的ケア、スピリチュアリティの表出を支えるケア、患者との関係保持が関連を示した。介護職員に対する満足度には、介護老人福祉施設では利用前の関係性、利用中の関係性、介護をやり切った感、施設での看取り、施設での葬儀、基本的ケア、スピリチュアリティの表出を支えるケアが、介護老人保健施設では施設での葬儀、基本的ケア、スピリチュアリティの表出を支えるケアが関連を示した。ケアに対する満足度では、介護老人福祉施設では施設での看取り、施設での葬儀、基本的ケア、スピリチュアリティの表出を支えるケアが、介護老人保健施設では施設での看取り、基本的ケア、スピリチュアリティの表出を支えるケアが関連を示した。以上より、介護老人福祉施設及び介護老人保健施設におけるスピリチュアルケアや宗教的な関わりの実践について、利用者家族からは施設の満足につながることが示唆された。今後、施設と外的資源との連携を図ること、介護職員への看取りの教育や実践の質をより高めることが、施設利用の満足度を高める上でもより重要であると考えられた。
