项目摘要

オランダの「インフォーマル・ケア専門知識センター」が手掛け、オランダ政府保健福祉スポーツ省の助成により出版された「インフォーマル・ケア・スキャン・エイド」について、介護や福祉の専門職が実践でそのツールをどう用いるか調べた。このツールは、要介護者や介護者が、自らのインフォーマルなケア・ネットワークをマッピングし、構成員を緻密に位置付けるのを補助する道具である。それにより、支援ネットワークのもつ強さや弱さを把握する。この特徴は、介護を必要とする本人と介護者の面接から得られる情報を構造化する方法にあり、ケア・チームを強化・主導し、介護状況の力動を視覚的に把握するためにジェノグラムやエコマップを利用する。利用者のセルフ・マネジメントを中心におき、専門職の記録・保管に志向した発想とは異なる。インフォーマル・ケア・スキャンは、専門職が家族介護者やボランティアと協働するために用いられるツールであり、介護者が直面する負担の軽減を目的とする。そのステップ1は、家族を明確に把握する。ステップ2は、どのような介護が必要とされているかを把握する。ステップ3は、介護者がどのような状態か把握する。ステップ4は、本人を中心とした社会的ネットワークの規模・構造・機能を把握する。ステップ5は、これまでの情報をもとに、介護者が「できること」「望むこと」等を明記する。このステップを通して、インフォーマルな介護者が自らの状況を再認識し、ケア・ネットワークの構築に向け対処できるのを目指す。オランダの社会支援法には、市民自身が自らのネットワークを介護に活用することが明記される。その目的に沿って国、地方政府、専門職から市民に至るまで、一貫した動きのなかで、介護者支援の体制が整備される。日本でも、アセスメント・ツールが用いられるが、セルフ・マネジメントの発想を中心に据えたケア・ネットワークの形成には至っていない。
