项目摘要

研究期間延長をうけて実施予定であった人口減少・高齢化が進展していると思われる北海道郡部における主要二次救急医療機関の救急部と同医療圏の消防本部への訪問調査の実施を改めて模索した。しかし2022年度は2021年度以上の新型コロナウイルス感染症の蔓延・拡大が続き、調査対象の医療機関や消防本部がその対応に直面・奔走していることを踏まえ、やむなく調査の実施を見送った。その一方で、各種学協会における発表や専門誌などの文献による情報収集に引き続き努め、新型コロナウイルス感染症（疑い）傷病者の救急医療の現場での対応の課題を検討した。そして、①検査の実施・感染確定まで一時待機する空間や感染患者を診療する空間の必要性、②感染患者の画像診断部・専用病棟等への動線の確保、③感染リスクや害悪性等の評価の経時変化に伴う診療・対応方法の可変性、などの課題を抽出した。本研究対象である郡部における二次救急医療機関では感染患者受入れの指定を受けている病院も多く、これら課題に相応した救急部の運営や建築設備の整備のあり方を運営体制や救急医療が展開される空間の規模の差異も勘案しながら検討する意義は大きいと考えた。なお次年度は、新型コロナウイルスの法的位置づけが変更する予定であることから、調査未実施の医療圏における二次救急医療機関や消防本部を対象とした調査の実施と共に、感染症指定医療機関の救急部を対象とした検討などを重ねて、最終的な成果を取り纏める予定である。
