项目摘要

研究対象者に、本調査のために作成したパンフレット「過覚醒状態についてのセルフチェック―心の余裕度を観察しましょう―」を用いて説明し、過覚醒状態（心の余裕度）についてセルフチェックを行ってもらった。1～2週間に1回の頻度で継続的にインタビューを実施し、セルフチェック結果を研究対象者に話してもらった。この調査を、2か月間、3名の対象者に対して実施した。他の1名は、4か月間の調査を実施した。うち2名については、引き続き令和5年度も調査を継続している。過覚醒状態について表現した対象者の語りの分析結果1名分を報告する。対象者は、「相手に対して怒り・恨み・嫉妬などの感情が湧き、伝わってしまうと感じる」時に過覚醒状態になると話し、過覚醒状態を『頭が一杯一杯になる』・『バランスがめちゃくちゃになる』・『恐怖』・『パニック』と感じ取っていた。また、「聞こえてくる音に自分に関する意味を感じる」時に過覚醒状態になると話し、過覚醒状態を『頭が痛い』、『心が暴れる』と感じ取っていた。「内的な思考が活発になる」時にも過覚醒状態になると話し、過覚醒状態を『心の中が変になる』・『わからなくなる』と感じ取っていた。これらコントロール不能感・恐怖感・過敏さ・頭痛・心の変調感を、過覚醒状態をとらえる手がかりとして利用することができるか、研究グループで検討した。その結果、これらの表現が、精神症状によって体験している感覚と重なるため、覚醒レベルを捉えるためには有用ではないという判断となった。そこで、0～10の11段階のスケールで覚醒レベルを査定する方法を用いて調査を継続することになった。この対象者が過覚醒になるのは、精神症状が生じた時と、家族への欲求不満が生じた時であった。今後は、過覚醒状態を鎮静する対処が効果的であった場合とそうではない場合、それぞれのその後の精神症状変化についてさらに詳細に調査・分析を行う。
