项目摘要

本研究の目的は、小児プライマリケアの視点にもとづく小児救急看護実践ガイドの開発を行うことである。文献検討では、救急外来を受診する子どもは、緊急度の低いケースが中心であることが再確認された。子どもの症状に応じて受診する保護者がいる一方で、不安による受診もあった。加えて、保護者の想定とは異なり入院に至るケース、つまり、親が子どもの症状を実際よりも軽症に見積もっていたということである。看護師は、子どもの身体状態から緊急度を判断するだけでなく、家族の状況を確認した上で、親子に必要な関わりを行っていた。親の理解度や見立て方、養育能力や心理的な状態を含み、アセスメントしていた。また、虐待の存在を意識しながら情報収集し、必要に応じて関わっていた。家族を支援する体制の具現化は発展の余地があることが示唆された。また、海外の文献を含めたスコーピングレビューの結果、日本では予防的なケアが行われていなかった。加えて、海外では、患者教育や指導の方法も多岐にわかり、ICTの活用やオンライン化もみられた。これらの結果から、小児プライマリケアにおける看護を、予防、教育、ケア調整の3つに大別し、それぞれの具体的な内容の構築を進めている。それぞれの下位項目として、予防ではポピュレーションアプローチとしての疾患予防や事故予防、育児支援、教育では、ハイリスクアプローチとしての疾患の再発予防、事故再発予防、虐待対応、ケア調整では、多職種多機関連携、移行（期）ケアを原案としている。また、ICTを活用したアプローチ方法も検討に加える予定である。
