项目摘要

本研究は、いまだ法制度化されていない全国の五歳児健康診査の実施状況を把握しその促進要素を探索することである。前年度は、五歳児健康診査を行っている近隣市町村3か所からその実施内容を把握した。今年度は、前年度把握した内容と研究者間での話し合いにより調査票を作成し、全国の保健センターを対象に調査を実施した。特別区と政令指定都市を省く全国市町村保健センター1741件を対象とした。回収は809通 46.5％。結果は、「５歳児全員を対象とした５歳児健康診査の実施」は123件（15.2％）、「定期的な５歳児健康相談の実施」82件（10.1％）、「特定の項目検査の実施」28件（3.5％）、「不定期の５歳児健康相談の実施」13件（1.6％）、「その他」125件（15.5％）であった。その他の具体的な内容は「保育所内科・歯科健診後の相談（必要時）」という保育所や幼稚園と連携をしたものや「五歳児の発達相談」という発達に焦点化した内容が多かった。５歳児健康診査を行う上で最終判断をしている職種は、「保健師」145件が最も多く、次いで「臨床心理士」85件、「小児科医」45件、「小児発達専門医」33件、「医師」14件の順であった。保健師が最終判断をしていることが多いことから、今後の５歳児健康診査の実施については保健師・臨床心理士等のパラメディカルでの実施の可能性も視野に入れることが可能と考える。今後は法制度を含めての検討が必要である。保健師が発達の正常を判断する方法としては、「厚生労働省児童家庭局長通知の「乳幼児に対する健康診査の実施について」実施要綱に沿って実施」「各県で作成した診察項目を使用して独自に作成した」「SDQ」「医師会が作成した項目」と、各自治体により発達の評価が異なっており全国での基準の統一が必要と考える。昨年度は一自治体の保健師を対象に乳幼児期の発達を評価する経験を調査した。
