本研究は、地域住民を対象として、中心動脈圧・Augmentation Index（AI）、血流依存性血管拡張反応: FMD検査による％FMD（拡張率）と循環器疾患の発症リスクとの関連を検討することを目的とする。地域において30～79歳男女を対象とし、約5,000人に中心血圧・AIの測定し、約2,000人に血流依存性血管拡張反応検査により％FMD（拡張率）を実施した。その結果、喫煙者の平均FMD値は、非喫煙者と比べて有意に低値を示し、30本/日以上吸っている喫煙者、大量喫煙者、そして長期喫煙者でFMD低値の頻度が高い傾向を示された。また、FMD値の4分位とし、FMD最高値群を基準とし、多変量調整したオッズ比を計算した。その結果、FMD高値群に比べ、FMD最低値群の腎機能低下の多変量調整オッズ比は有意に高かった。上記の関連は降圧剤服薬者を除いた解析においても同様な結果を示した。血管内皮機能の低下と腎機能障害と関連することを示された。さらに、2010～2012年に40～79歳男女を対象とし、中心血圧・AIの測定し、循環器疾患による受診歴のない男女4,167人を解析対象とした。中心血圧値を低値群，中央群、高値群; AI値を低値群，中央群、高値群の3群に分類した。2019年末までに平均6.8年の追跡期間中に、男女70人が全循環器疾患に発症した。全循環器疾患発症の年齢、性別、地域調整ハザード比は、中心血圧の低値群と比べて、中央値群では2.60、高値群で3.07であり、AIの低値群と比べて、AIの中央値群では1.25、AIの高値群で0.82であった。これらの関連は多変量調整しても関連に大きな変化がなかった。多変量調整したハザード比はそれぞれに中心血圧値の中央群と高値群では2.56と2.93であり、AI値の中央群と高値群では1.30と0.85であった。

The purpose of this study is to investigate the relationship between population, central arterial pressure, Augmentation Index (AI), blood flow-dependent vascular tension, FMD, and circulatory disorders.
