本研究の目的はシスプラチン投与を受ける我が国の頭頸部癌症例において、アルコール代謝関連遺伝子の多型と飲酒量の低マグネシウム血症への寄与を定量する ことである。シスプラチンは頭頸部癌の標準治療薬であり、用量制限毒性は腎毒性である。従って腎機能は頭頸部癌における一種の予後規定因子と考えること が出来る。研究代表者は平成28年度に頭頸部癌患者においてマグネシウムを補充することでシスプラチン投与量を有意に多く維持可能であることを論文報告し た。低マグネシウム血症は一般的には飲酒、下痢、低栄養、プロトンポンプ阻害剤などが原因とされている。一 方、飲酒による発癌性は遺伝子多型によって大 きく異なることなり、中でもアルコール代謝に関わるALDH2/ADH1遺伝子多型の影響が大きいことが知られている。 自験例では大量飲酒例で高度な低マグネシウ ム血症を来していた。以上から「低マグネシウム血症もALDH2/ADH1遺伝子多型や飲酒量の影響を受ける。」と仮設を発案するに至った。 まず必要なサンプルサ イズを見積もるためのパイロットスタディを行った。報告書記載時点で20例ほど遺伝子検査を行っているが、マグネシウム低値の症例数が前施設のデータより少 なくパイロットスタディを終了できていない。 パイロットスタディと平行して遺伝子多型以外に治療を妨げる因子の抽出を行っている。シスプラチン同時併用化学放射線療法は局所進行頭頸部癌の標準療法で ある。本治療においてはシスプラチン投与量が多いほど予後良好であることが示されているが、有害事象により途中で投与を断念することも少なくない。粘膜 炎、腎機能障害、誤嚥性肺炎が高度になると緊急入院や長期入院を来す。当院患者では緊急入院はN1以上の症例で多く、長期入院は局所進行例で多かった。その 後、緊急入院の対策として予防的胃瘻の作成を進めており、今後再度の解析を行う予定である。
