项目摘要

本研究では腎移植レシピエントにおける性の健康、QOLの維持・向上につなげるため、腎移植レシピエントの性機能およびQOL、性機能を取り巻く要因との関連を明らかにし、腎移植レシピエントの性の健康を支援するアプローチモデルを開発している。研究デザインは横断研究である。腎移植実施施設に研究協力を依頼し、腎移植を受けた患者116名(男性73名、女性43名)に対して研究協力の説明ならびに協力依頼を行った。調査内容は基本属性、性機能の評価として男性はInternational Index of Erectile Function (IIEF)、女性はFemale Sexual Function Index (FSFI)、健康関連QOLの評価としてSF-36、疲労の評価としてFunctional Assessment of Chronic Illness Therapy-Fatigue(FACIT-F)である。本研究に対して同意が得られたのは111名(男性72名、女性39名)であり、調査票への回答が得られたのは83名(男性53名、女性30名)(回答率71.6%)であった。共分散構造分析で腎移植を受ける患者の性機能およびQOL因果関係の初期モデルを男女別で構築するにあたり、令和４年度は得られたデータについて単変量解析および多変量解析を実施した。結果の一部として、血清クレアチニン値および雇用形態は、FACIT-Fatigue Scaleスコアと関連は認められず、腎移植レシピエントは自身の健康状態を管理するために、勤務時間や勤務内容を調整している可能性があると考えられた。今後は、得られた結果をもとに腎移植レシピエントの性の健康に対する支援について論文としてまとめる。

This study aims to clarify the relationship between sexual health, QOL maintenance, and sexual function in kidney transplantation, and to explore the role of sexual health support in kidney transplantation. The study is a cross-sectional study. 116 kidney transplant recipients (73 males and 43 females) were enrolled in this study. The survey included basic attributes, evaluation of sexual function, male International Index of Erectile Function (IIEF), female Sexual Function Index (FSFI), health-related QOL, SF-36, and Functional Assessment of Chronic Illness Therapy-Fatigue(FACIT-F). 111 respondents (72 males and 39 females) agreed to the survey and 83 respondents (53 males and 30 females) responded (response rate 71.6%). Co-distribution structure analysis of sexual function and QOL in patients undergoing renal transplantation. The results showed that there were some differences between the serum concentration and the employment pattern, and the FACIT-Fatigue Scale was related to the management of the kidney transplant patients 'health status, service time and service content. In the future, the results of kidney transplantation will be better.
