本研究では、茨城県内の在宅医療を行う複数の医療機関と連携し、「マルチモビディティを持つ在宅療養患者の健康関連アウトカムに影響する要因を網羅的に探索し、処方内容や多職種連携によるサービスの質が、健康関連アウトカムにどのような影響を及ぼすか」を明らかにすることを目標としている。初年度において、現場の負担感を考慮した実現可能性の観点から、研究計画の見直しが迫られたため、2022年度においては、パイロット調査のための研究計画書の策定、倫理委員会への申請、データ収集の実施などを行った。2022年3月より自施設でのパイロット調査を実施した。パイロット調査では自施設で訪問診療を行っている成人約100名を対象として、主治医による慢性疾患の評価を行い、そのパターンを分析し、医師の診療負担感・診療時間との関連性を評価することとした。データ収集は終了し、現在データを分析中である。これらの調査結果は今後の研究計画を再検討するための基礎データになると考えられる。2022年度の研究費使用は主に学会などでの情報収集、研究実施の準備、データ入力などに使用した。学会でのネットワーキングにより、課題周辺の研究を行っている研究者との意見交換を行うことができ、研究計画のブラッシュアップにつながった。また、健康関連アウトカムの尺度については、QOLや治療負担感の測定ならびに回答が困難な患者の代理評価などが課題であり、健康関連アウトカムの尺度については、引き続き検討することとした。

This study aims to explore the main factors affecting the health-related outcomes of multiple in-home care providers in Ibaraki Prefecture, including prescription content, quality of care, health-related outcomes, and health risks.
