私達は、「双極性障害(BD)に対する維持期治療でより有用性の高い薬を系統的レビュー(SR)とメタ解析を用いて探索し、それらの薬の急性期治療における有用性を既報のガイドラインを基に評価し、急性期から維持期にかけて連続性のあるBD治療を提案したい」と本研究を企画した。本研究では以下の4つの臨床疑問に対する答えの創出を目指した。臨床疑問①：維持期BD患者に対し、どの抗精神病薬(AP)および/または気分安定薬(MS)が優れているかをSRとネットワークメタ解析を用いて調査した (研究数=41本、患者数=9821人)(Kishi 2021)。オランザピンとクエチアピンは全ての気分エピソード・うつ病エピソード・躁病/軽躁病/混合エピソードの再発率と全ての理由による治療中断率がプラセボより低かった。ただし、オランザピンとクエチアピンはそれぞれ傾眠と口渇のリスクがあった。臨床疑問②：MSか第二世代APで臨床的に安定した維持期BD患者は、どのくらいの期間、薬物治療を中止すると、それらを維持した場合に比べて再発率が高まるのかをSRとメタ解析を用いて検討した (研究数=22本、患者数=5462人)(Kishi 2021)。臨床的に安定したBD患者において、薬物治療を維持することは、最大24ヶ月間再発を防いだ。さらに、1か月間以上の薬物治療の中止は、再発リスクを大幅に増加させた。臨床疑問③：リチウムを継続した成人BD患者における実際の再発率を、シングルアームメタ解析を用いて調べた(研究数=21本、患者数=1415人)(Kishi 2020)。全ての気分エピソードの再発率は39.8％、うつ病エピソードの再発率は25.6％、躁病/軽躁病/混合エピソードの再発率は18.5％であった。臨床疑問④(追加)：維持期BD患者に対するAPおよび/またはMSの最適な用量を用量反応メタ解析を用いて検討する。
