膵内外分泌機能低下、膵発癌の原因として近年脂肪膵が注目されている。その機序については膵への脂肪沈着によって引き起こされる炎症、いわゆる脂肪膵炎が原因の一つとして予測されている。しかし脂肪膵炎については仮説レベルであり、動物実験モデルなどでの検証は未だなされていない。本研究では脂肪膵炎動物モデルの構築と、同モデルを用いた脂肪膵における膵内外分泌機能低下、膵発癌の機序を明らかにすることを目的とした研究である。まず本研究では最適な脂肪膵炎モデルの構築のため、高脂肪食を摂取させたマウスに慢性膵炎を惹起させるために用いられるセルレインの反復投与を行うこととした。まずは野生型マウスとしてC57BL/6J、発癌モデルマウスとしてK-RASノックアウトマウスを採用した。それぞれのマウスで介入法としてPBS or セルレイン反復腹腔内投与、食事として標準食 or 高脂肪食にそれぞれグループ分けを行った。それぞれの膵病理検体を評価したところ、膵の炎症の程度や膵癌の早期段階と考えられるPAN-INの発生状況としてはK-RASノックアウト＋高脂肪食＋セルレイン反復腹腔内投与が最も重症度・腫瘍発生率が高い傾向にあった。一方でセルレインの投与間隔については既報を参考に投与間隔や投与量の調整を行っているが、セルレイン投与量と膵炎重症度については用量依存的な関係は不安定であり、またセルレイン反復腹腔内投与＋高脂肪食群では他群と比較すると最終投与日までに死亡する個体も多かったが、必ずしも膵炎との重症度との相関関係が強いという訳ではなかった。実臨床を考慮すると、脂肪膵炎と呼ばれる状態が必ずしも急性期の重症膵炎ということを指してる訳ではあいため、生体内環境を反映した膵炎の設定を行うことが重要である。
