本研究の目的は, 冠循環局所における各種血管作動性物質の動態と血管内皮および平滑筋機能障害との関連性を解析し, 冠動脈機能異常の詳細な機序を明らかにすることである. この目的を達成するため, ①冠循環局所における血管作動性物質および関連バイオマーカー (セロトニン, Rhoキナーゼ活性, VCAM-1など) の動態, ②血圧や臓器血流を規定する抵抗血管における血管機能不全の成因を明らかにすることで, 冠動脈機能異常の成因と機序に基づく新たな治療戦略の開発につなげる. 具体的には以下のBed-Bench連携で研究を継続した.初年度 - 第3年度：Bed 包括的な冠動脈機能評価 (冠攣縮誘発試験, 冠静脈洞採血など).初年度 - 第3年度：Bed-Bench 冠循環局所におけるバイオマーカー動態解析.第2年度 - 第3年度：Bench 摘出血管標本を用いた血管機能実験.本研究内容に関して本学内の倫理委員会での審査を経て承認された. 倫理委員会で承認後, 初年度は6名, 第2年度は23名の患者に本研究に参加頂き, 包括的な冠動脈機能評価を実施した. まだ登録患者数が少なく, 結果の解析は未実施であった. また, 事項で述べるように, 冠動脈機能異常など関連テーマに関して総説論文を発表し, 関連する病態を取り扱った日本循環器学会ガイドラインの改訂作業に協力員として参加した. これら総説論文やガイドラインでは, 申請者らのこれまでの研究成果に基づき, これまでに明らかにされてきた冠動脈機能異常の疫学, 分類, 成因, 臨床的意義, 今後の展望などについて, 概説した. さらに, 関連した病態に関して冠動脈機能異常の症例報告論文を発表した.

The purpose of this study is to analyze the relationship between the dynamics of vascular motile substances and vascular endothelial and smooth muscle dysfunction in coronary circulation, and to clarify the detailed mechanism of coronary artery dysfunction. To achieve this goal,(1) the dynamics of vasoactive substances and related factors (such as coronary circulation, Rho activity, VCAM-1),(2) the regulation of blood flow in blood pressure vessels, and (3) the causes of vascular insufficiency, and (4) the mechanisms of coronary dysfunction, and (5) the development of new therapeutic strategies. Specific Bed-Bench Connection Research. Year 1-Year 3: Coronary artery function evaluation (coronary contracture induction test, coronary vein hole blood sampling) included in Bed. Year 1-Year 3: Bed-Bench crown cycle dynamic analysis. Year 2-Year 3: Bench plucked blood vessels and used them for vascular function. The contents of this study are related to the review and approval of the Ethics Committee within the study. After the Ethics Committee's recognition, 6 patients participated in the first year and 23 patients participated in the second year. The number of registered patients is small, and the analysis of the results is not implemented. The results of this study are based on the findings of the applicant, and the epidemiology, classification, causes, clinical significance of coronary artery dysfunction and future prospects are summarized. A case report on coronary artery dysfunction is presented.
