ABO血液型不適合生体腎移植では、術前に抗血液型抗体価を十分に低下させるための強力な脱感作療法が必要不可欠である。しかし、抗体除去療法を施行したに関わらず、すぐに抗体除去療法前の抗血液型抗体価まで再上昇してしまういわゆる『リバウンド現象』はリツキシマブ時代となった現在においてもしばしば遭遇する事象である。そこで、我々は、秋田大学で2006年～2022年までにリツキシマブ（200mg/body）投与後にABO血液型不適合性遺体腎移植を施行した85例を対象にリバウンド現象と液性拒絶の関連及びリバウンド現象の発症機序について研究を行った。ABO血液型抗体価のリバウンド現象を認めた症例は22例（26％）であった。リバウンド現象が起こる術前の危険因子は、ベースラインの抗血液型抗体価128倍以上（p<0.001, 7.066［2.257-22.121］）であった。また、移植後1カ月以内に液性拒絶反応を発症した症例は12例（14％）であった。液性拒絶反応のリスク因子の検討では、治療開始前や移植直前のABO血液型抗体価は有意な危険因子ではなかったのに対して、抗体価リバウンド現象は液性拒絶反応の有意な危険因子であった（p=0.005)。また血清BAFF濃度(B cell activating factor belonging to the tumor necrosis factor family）は、移植前日と術翌日で有意に抗体価リバウンド症例で高値であった。このことから、抗体価リバウンドにBAFFが影響している可能性が示唆された。さらに現在移植術時に採取した2次リンパ組織を使用して、CD20、CD138などのB細胞レパートリー染色を行っており、リバウンド現象との関連性を探索中である。

ABO blood type is not suitable for living kidney transplantation, but the anti-blood type antibody level is very low before operation. The anti-blood antibody level rises again before antibody elimination therapy is performed. From 2006 to 2022, Akita University conducted research on the relationship between fluid rejection and the mechanism of fluid rejection in 85 cases of ABO blood type incompatibility cadaveric kidney transplantation after administration of 200mg/body. ABO blood type antibodies were detected in 22 cases (26%). Preoperative risk factors were 128-fold higher (p<0.001, 7.066[2.257-22.121]) than those of preoperative risk factors. 12 cases (14%) of the patients refused to accept the treatment within 1 month after transplantation. The results showed that ABO blood type antibodies before transplantation and before treatment started were significantly higher than those before transplantation (p=0.005). Serum BAFF concentrations (B cell activating factor belonging to the tumor necrosis factor family) were high on the day before transplantation and on the day after transplantation. The possibility that BAFF will affect the immune system is demonstrated. In recent years, we have been exploring the relationship between the use of CD20, CD138 and B cell staining in transplantation.
