2022年度は、蓄積された組織標本およそ５０例について免疫組織化学染色によりSF３B1の発現率（H -score）の評価を行った。まず全体で、術前患者データとの関連を検討したが、患者背景が多岐に渡り、年齢層が異なる場合に同じ評価項目を用いることが困難であるなどあったため、まず、対象を「初回心内修復術を行った２５例」に限定して解析を行った。Hーscoreと関連する術前の血行動態指標が明らかとなり、右室・左室負荷がHーscore発現亢進と関連した。この検討結果は、2022年10月に行われた日本胸部外科学会学術集会において口演発表の機会を得られ、こちらは、優秀ポスター賞を受賞に至った。これに、同年度に追加で染色・評価を行った標本を加え、n=50として再度解析を行い、主な結果や傾向についてさらに明確な結果が得られており、論文投稿に向けて準備を進めている。また、同年の医学研究実習においても研究テーマとして取り上げ、今度は、再治療介入を行った２３例にて同様に解析を行った。初回心内修復群が主に乳児や幼児期早期であるのに対し、ある程度年齢の進んだこちらの群では、H-scoreの発現傾向に違いが認められた。どこかで心負荷にともなってHーscoreの発現は低下し始めるのではないかと推察された。いずれも、まだnが少ない状況で、術後経過と合わせてのリスク評価には至っていない。今後、症例を重ねた上で、nを増やした状況あるいは疾患群をそろえてある程度のnを形成し、心不全の改善までを視野に入れた検討ができると良いと考えているところである。

In 2022, we evaluated the H-score of SF3B1 by immunohistochemical staining in 50 cases of accumulated tissue. All patients were examined before surgery, and the patient's background was different. The same evaluation item was used in different cases. The analysis was limited to 25 cases of initial intracardiac repair. H score is correlated with hemodynamic parameters before surgery, and right ventricular and left ventricular loads are correlated with H score. In October 2022, the Japanese Society of Thoracic Surgery held an academic conference to discuss the opportunities for oral presentation, and the excellent awards were received. For example, in the same year, add dye, comment, line, n=50, analyze again, main result, tendency, clear result, submission, preparation, progress, etc. In the same year, medical research was carried out in 23 cases. The first cardiac repair group is mainly in the early stage of infancy, and the second is in the early stage of infancy. The heart load is low and the score is low. In the middle of the day, there is no change in the situation, the postoperative period, the evaluation of the disease, and the evaluation of the disease. In the future, the symptoms will be serious, and the conditions will be increased. The disease group will be formed, and the heart will be improved.
