RIPCに関する研究では，神経系因子と液性保護因子やそれらの相互作用が心保護効果に寄与することがすでに報告されており，申請者もRIPCの液性保護因子の1つに非アシル化グレリンの関与について報告している．しかしながら，全身麻酔薬のうち吸入麻酔薬の併用下ではRIPCの保護効果の発現さらには増強を認めているが，静脈麻酔薬のプロポフォールの併用下ではRIPCの保護効果は棄却されることが報告されているものの，その機序については未だ明らかになっていない．本研究の目的は全身麻酔薬併用下におけるRIPCの液性保護因子の1つである非アシル化グレリンの変動及び心保護効果について検討を行った．麻酔下で雄性Wistarラットに対して，RIPC手技としてラット後肢を血圧カフで5分間駆血し，その後5分間再灌流を3-4サイクル施行した．対照群(CON群)を作製した，各個体でグレリン値の変動を測定した．本研究ではグレリンの分泌と全身麻酔薬の一つであるセボフルラン投与下でのグレリン値はRIPC単独に比較し有意に上昇することが分かった．一方でプロポフォール投与下でのRIPCにおいても非アシル化グレリンの上昇を認めた．プロポフォールの心保護効果の棄却理由としてプロトノフォノアによるミトコンドリア膜電位変化に作用することが示されているため，RIPCによって非アシル化グレリンが上昇してもこの作用で心保護作用を示さないと推測された．

The protective effects of RIPC are enhanced in the presence of systemic anesthetics and inhaled anesthetics, and the protective effects of RIPC are eliminated in the presence of intravenous anesthetics and inhaled anesthetics. The purpose of this study was to investigate the effects of RIPC on the development of a variety of non-inflammatory and cardioprotective effects under general anesthesia. The male Wistar was reperfused for 5 minutes after the RIPC was performed. For the CON group, control is performed, and each individual is measured. In this study, we compared the RIPC with the RIPC, and found that the RIPC was significantly higher than the RIPC. One side of the RIPC is the rise of the RIPC, the rise of the RIPC and the rise of the RIPC. The reason for the rejection of the protective effect of RIPC on the heart is that RIPC has a protective effect on the heart.
