口腔顔面領域は身体の他の領域と比較して疼痛の発現頻度が高いと報告されている。歯科医療分野においては義歯を使用している者は多く、客観的所見と患者の訴えとの間に乖離を認めることがある。義歯に違和感を訴える患者の中には認知機能や口腔 感覚に変調をきたしている症例も存在する。本研究の目的は義歯の違和感を訴える症例の診断に対する客観的評価方法の確立である。申請者は過去に40歳以下の健常被験者に対して断眠を指示し、断眠が顎口腔領域の感覚機 能へ及ぼす影響を検討した。その結果、顎口腔領 域の体性感覚閾値は睡眠時間の減少によっ て有意に低下することを報告した。これまでに断眠による口腔領域の体性感覚機能の変調に関する 報告は少なく、申請者の過去の研究においては睡眠制限により口腔領域の痛覚閾値は低下し、敏感となる傾向が認められた。よって睡眠の客観 的評価や、睡眠の中枢機能への影響、認知機能の変化による睡眠への影響、認知機能と口腔機能との関連性を検討しているため、本研究により 得られるデータと比較検討が可能である。本研究では65歳以上 の義歯を使用している被験者に対して睡眠制限を指示する。測定項目は睡眠評価、ミニメンタルステート検査、口腔機能精密検査、定量的感覚 試験、心理社会的様相、唾液中のストレスマーカーの測定を行い、各項目をスコア化する。義歯の違和感のある群、義歯の違和感のない群およ び睡眠制限の有無の4群比較を行い、睡眠状態が認知機能や口腔感覚へ及ぼす影響を検討する。 よって本研究において睡眠状態と口腔領域の体性感覚および認知機能、口腔機能の関連を検討することで、長期にわたり義歯への痛みや違和 感の治療を必要とする患者に対する客観的な診断方法の策定および治療方針の一助となると考えられる。
