医療者の助言に反して入院を中断、退院(DAMA: discharge against medical advice)し、重症化した状態で再受診、再入院に至るケースがあり、死亡率や合併症の上昇、医療費の増加につながるとともに、医療現場の混乱を招くため、世界的に問題視されている。これまで日本における DAMA に関する調査は行われていないため、本研究では、群馬県における DAMA の発生頻度や発生要因を調査することで、日本における DAMA の現状を調査し、医療的、社会的な課題かを明らかにすることを目的とし、臨床研究を開始した。まず、DAMAに至った症例の発生要因を分析するため、患者背景、病名、検査・治療内容、入院までの受診回数、入院日数、再診・再入院率、DAMA後の転帰や治療経過を観察項目とする質問紙票の作成をした。また、質問紙票を配布する協力依頼機関として、専従あるいは専任の医療安全管理者を設置している群馬県内の医 療機関を抽出した。本研究について、医師主導臨床研究計画書を作成し、群馬大学医学部附属病院の臨床研究審査委員会に申請した。令和3年7月29日に承認を取得した。これまで、抽出した協力依頼機関の中で、約半数の医療機関に研究協力を依頼した。協力を得られた各医療機関において、臨床研究開始の手続きを経て、調査を 実施した。今年度前半までに、残る半数の医療機関へ調査協力を依頼し、DAMAの実態調査を完了する予定である。その後、調査結果の分析・評価を行い、最終的には論文化、学会発表を目指す。
