项目摘要

本研究は、要介護高齢者の重度化予防に関して、個人の社会的関係性の影響の検討および居住地域の社会関係要因の影響の検討を行い、要介護高齢者のウェルビーイングの向上とそれに資する地域づくりの提言を行う。2022年度は、昨年度にひきつづき「日米LTCI研究会東京・秋田調査」の二次データ分析を行い、個人の社会関係として孤食に着目し、孤食がその後の要介護の重度化と関連することを明らかにした。また、地域環境要因として高齢者にやさしいまち（Age-Friendly Cities：AFC）の尺度開発にも取り組み、分析結果と合わせて要介護高齢者のウェルビーイング向上に対する社会的要因として、学会でのパネル発表および総説としてまとめた。また、要介護高齢者に関して2つの新規コホートデータの構築にも取り組んだ。まず、軽度要介護高齢者のコホートとして、自治体と共同し、対象者名簿の提供など調査実施枠組みの調整を行った。また併せて調査設計を進め、2023年度には訪問面接調査によるデータ取得を予定している。公的データとの突合から重度化アウトカムの収集を行い、個人および地域の社会的要因による重度化予防の可能性について検討を進めていく。また、自治体が実施する在宅介護実態調査の枠組みを活用し、千葉大学等と共同し、郵送質問紙調査による「在宅ケアと暮らしの調査」を実施した。3000名弱の要介護高齢者のデータを取得し、今後は転帰データとの突合を含め、重度化予防に資する個人の社会的要因の検討やAFCも含めた地域要因の検討を行っていく。
