项目摘要

令和4年度は本課題へ直結する研究成果として、「先天性心疾患児の社会適応スキルとその関連要因の検討」を学会発表した。この中で学童期を通じて先天性疾患児は社会適応スキルの発達を健常児同様に経ていたこと、その一方で高学年児においては、内服薬や外来通院頻度といった疾患に関連する要因が言語スキルに関連していることが明らかになった。また学会論文として「The association between the social adaptive skills of school-aged children with congenital heart disease and mothers' decision factors for providing disease explanations in Japan: a cross-sectional study」が掲載された。この中では母親が子どもに病気説明を行う際の決定要因は、母親の説明能力、子どもの説明への受容能力、説明に対する子どもの興味とリテラシーであること、言語スキルと社会生活スキルは子どもの説明への受容能力と有意に関連し、言語スキルと日常生活スキルは説明に対する子どもの興味とリテラシーと有意に関連することが示された。これらの成果から、先天性心疾患をもつ子どもの社会適応能力は疾患との関連性を示しつつも維持され、さらに病気概念形成に大きく影響を与える母親からの病気説明に関連することが示唆された。これは今回の研究計画を進行していくにあたり、非常に有効な要素になり得ると考えられる。また今年度までに行った文献検討から、病気概念形成はその先で子どもの自己概念形成に寄与することが予測できている。さらに思春期から青年期にかけての自己概念形成においては、【家族の中の自己】【社会の中の自己】【病気をもった自己】という３つのテーマを導き出すに至った。
