本研究全体を通して,これまで評価されていなかった随意的な筋弛緩を伴う運動課題と認知課題からなる二重課題を同時に行う際の随意運動の発現過程さらにその時の二重課題干渉を,皮質脊髄路の興奮性の指標であるMEPならびに皮質抑制性の指標であるCSP durationという神経生理学的指標を用いて評価するための研究計画を立てた。本研究では,随意運動の発現に大きく関与している,一次運動野(primary motor cortex: M1)からの運動指令の伝導路である皮質脊髄路の興奮性を経頭蓋磁気刺激法(transcranial magnetic stimulation: TMS)を用い,筋から誘発される運動誘発電位(motor-evoked potential: MEP)振幅ならびに皮質抑制性を反映するCSP durationを指標にすることで非侵襲的に評価する。本研究は,随意的な筋弛緩を伴う運動課題と認知課題からなる二重課題の遂行が皮質脊髄路の興奮性に与える影響(実験①),随意的な筋弛緩を伴う運動課題と認知課題からなる二重課題の遂行が皮質抑制性に与える影響(実験②),二重課題のトレーニングが皮質脊髄路の興奮性ならびに皮質抑制性に及ぼす影響(実験③)を検討する,3つの実験で構成される。実験中は参加者には,安静座位状態で1)安静条件,2)認知課題条件,3)10%MVCと30%MVCから音に合わせて完全脱力の状態にする(0%MVC)課題(単一運動課題条件),4)3)の課題と同時に2)と同様の認知課題を遂行する課題(二重課題条件)を行ってもらう。それぞれの課題において,左運動野(M1)をTMSで刺激し,右手第一背側骨間筋(FDI)から 皮質脊髄路興奮性の指標となるMEPを記録することとした。令和4年度の進捗としては,上述3つの実験課題を遂行するための実験システムの確立まで完了している。
