项目摘要

甲状腺乳頭がんのHobnail亜型は再発や遠隔転移の頻度が高く予後不良で、未分化転化と関係し原病死につながる。Hobnailパターンの形態学的特徴は、がん細胞の極性の乱れと細胞接着の異常(LOP/LCC)を反映し、転移・浸潤能の獲得につながる上皮―間葉転換(EMT)を示唆する。97例の原発性甲状腺がんを解析し、16例のBRAFV600EとTERT promoterのdouble mutantを検出し、その全てで浸潤部にLOP/LCCパターンを認め、14例がpT3 or 4、10例がpN1で、9例がKi-67標識率6% 以上であることが判明した。これらを対象とし、通常型乳頭がん成分では正常濾胞と比較してCK8/18とClaudin-1発現が亢進し、VimentinとZO-1発現の極性(Apical-basal polarity)は保持され、予後不良亜型である高細胞亜型にはβ-cateninの異所性発現(核上部胞体内発現)やClaudin-1とVimentin不均一発現が、Hobnail亜型成分にはVimentin, Claudin-1, ZO-1、β-cateninの発現低下が関与し、さらに低分化がんにはZO-1発現は保たれApical-basal polarityは維持されているが、CK8/18とVimentinの発現が消失することが判明した。これらの結果は、甲状腺がん進行に関連する組織構築の変化過程には細胞骨格と細胞間接着のダイナミックな変化が関与することを示唆していて、細胞接着性や中間フィラメントの調節異常が高分化がんの予後不良亜型や低分化がんの形態形成に重要と思われる。
The frequency of recurrence and distant migration of thyroid papilla is high, and the relationship between poor differentiation and death is high. The morphological characteristics of Hobnail's skin include: cell polarity disorder, abnormal cell adhesion (LOP/LCC), epithelial mesenchymal transition (EMT), and migration and infiltration. 97 cases of primary thyroid were analyzed, 16 cases of BRAFV600E and TERT promoter were detected, all cases of LOP/LCC were detected, 14 cases of pT 3 or 4, 10 cases of pN 1, 9 cases of Ki-67 were detected, and the identification rate was more than 6%. The normal nipple component is opposite to the normal filter cell. The CK 8/18 Claudin-1 appears opposite to the normal nipple component. The Vimentin ZO-1 appears opposite to the normal filter cell.(Apical-basal polarity) is maintained, and after the bad type, the high cell type is the β-catenin and the heterosexual development.(In the upper part of the nucleus) Claudin-1 and Vimentin heterogeneously occur, Hobnail-type components Vimentin, Claudin-1, ZO-1, β-catenin occur less frequently, ZO-1 occur less frequently, Apical-basal polarity remains less frequently, Hobnail-type components occur less frequently, ZO-1 occur less frequently, Apical-basal polarity remains less frequently, Hobnail-type components occur less frequently, Hobnail-type components occur less frequently, ZO-1 occur less frequently, ZO-1 occur less frequently, Apical-basal polarity remains less frequently, Hobnail-type components occur less frequently, Hobnail-type components occur less frequently, ZO-1 occur less frequently, ZO-1 occur less frequently, Apical-basal polarity remains less frequently, Hobnail-type components occur less frequently, Hobnail-type components occur less frequently, Hobnail-type components occur less frequently, ZO-1 occur less frequently, ZO-type components occur less frequently, ZO-1 occur less frequently, Apical-basal polarity remains less frequently, Hobnail-type components occur less frequently, Hobnail CK 8/18: Vimentin's appearance disappeared.
