高二倍体急性リンパ性白血病は、様々な遺伝学的特徴を持つヘテロな集団であり、一般に予後良好とされているが、再発症例は非常に難治で予後不良となる症例が多い。ゲノムおよびエピゲノム解析で、再発・難治症例の特徴および治療抵抗性獲得のメカニズムを解明することを目標としている。研究採択前に再発・難治症例3例と予後良好症例4例のDNA抽出を行ない、解析を開始していた。2022年度は予後良好症例を5症例追加し、過去の症例を含めてWES解析を開始した。コピー数多型の評価、および既存SNPデータを除外した上で、欠失遺伝子、変異遺伝子を評価した。経過良好症例群と、再発・難治症例の初発、1st再発、2nd再発までの検体を比較し、再発および治療抵抗性に関連する変異遺伝子を同定しようと試みている。
